Medical Needs Registry
The Medical Needs Registry is a database for Monroe residents, of any age, with special needs who would require assistance during an Emergency or Evacuation.

This confidential list, compiled by the Community and Social Services Department is updated annually, or when circumstances change, and is shared with Monroe’s Emergency Operations Center. Current information will ensure that appropriate services are provided in an emergency or evacuation situation.

Individuals with special needs would include, but are not limited to, hearing or sight impairment, bed confinement, wheelchair dependent, life support device, use of TDD/TTY, transportation needs or other conditions which would prevent prompt evacuation.

This confidential form is for individuals living in Monroe who need assistance during an evacuation or emergency for the reasons of hearing or sight impairment, bed confinement, wheelchair dependent, life support device, use TTD/TTY, other medical conditions or transportation needs that would prevent prompt evacuation in case of an emergency.

Each year we update our master list which is shared with the Town of Monroe’s Emergency Operations Center to ensure that appropriate services are provided in an emergency or evacuation.
